Part-Time Weekend Registered Nurse

This is a fully remote position, open to applicants in United States.

📋 Description

• Monitor patient vitals, symptoms, and lab results to identify individuals requiring clinical interventions.

• Conduct follow-ups with patients exhibiting abnormal readings to collect further details regarding their clinical status and triage as necessary.

• Address incoming patient clinical inquiries and escalations via phone and text message.

• Facilitate virtual follow-up sessions to assist with program enrollment, modifications to treatment plans, medication adherence, and achieving lifestyle objectives.

• Aid in clinical documentation and create care summaries for physicians overseeing patient care.

• Ensure that every healthcare interaction prioritizes patient welfare and aligns with the goals of the health system.

• Contribute to internal initiatives aimed at expanding care delivery for patients with CHF, hypertension, type 2 diabetes, and other chronic ailments.

• Help cultivate the culture of a rapidly growing Cadence team.

⛳️ Requirements

• A minimum of 4 years of experience in treating patients with CHF, hypertension, and diabetes in either outpatient or inpatient settings.

• Active compact multi-state RN licensure (RN compact).

• Experience working within a CHF bridge clinic environment.

• Familiarity with remote patient monitoring technologies.

• Capability to monitor patient vitals, symptoms, and labs to identify those who need clinical interventions.

• Proficiency in following up on abnormal readings and triaging as appropriate.

• Ability to lead virtual patient follow-ups that support enrollment, treatment modifications, medication adherence, and lifestyle objectives.

• A strong passion for enhancing patient/customer experiences and advancing healthcare through digital innovation.
