Part Time Licensed Practical Nurse, Weekends

This is a fully remote position, open to applicants in United States.

📋 Description

• Perform outbound telephonic outreach to patients and offer individualized coaching and support for those managing chronic illnesses.

• Assist patients in implementing tailored care plans and reaching their objectives through education and coaching in behavior modification, nutrition, physical activity, and self-management.

• Utilize motivational interviewing techniques and SMART goals.

• Conduct thorough assessments of patients’ health status, lifestyle choices, dietary habits, and willingness to change.

• Provide education to patients regarding disease management, medication compliance, symptom identification, and preventive measures.

• Carry out care delivery tasks under appropriate clinical supervision and according to established protocols.

• Track patient progress, adherence to treatment plans, and health outcomes through regular check-ins and remote monitoring tools.

• Record patient interactions, outcomes, and follow-up strategies in the Cadence platform.

• Recognize obstacles to care and connect patients with resources or internal support systems.

• Collaborate with clinicians and patient support teams using the Cadence Platform, Google Workspace, and Slack.

• Provide feedback to enhance workflows.

⛳️ Requirements

• A valid compact multi-state LPN/LVN license in the state of residence is mandatory.

• A minimum of 5 years of experience as a Licensed Practical Nurse is required.

• Preparedness for remote work (including reliable high-speed WiFi) is essential.

• An active LVN license in California is required.

• Proficiency in Spanish is preferred.

• Experience in providing education and support to patients managing chronic conditions is advantageous.

• Familiarity with AI-assisted clinical tools (like Gemini, Open Evidence, or similar platforms) and the ability to engage knowledgeably with providers and patients regarding the role of AI in care delivery versus the necessity of human judgment.

• Availability to work at least three days per week during standard business hours of either 8:00 AM to 5:00 PM or 9:00 AM to 6:00 PM Pacific Time.

• Willingness to work on Saturdays is required.

• Comfortable using the Cadence Platform, Google Workspace, Slack, and AI-assisted clinical tools.

• Reliable high-speed WiFi for remote work is necessary.
