Part-Time Care Coordinator, CMA

This is a fully remote position, open to applicants in United States.

📋 Description

• Conduct monthly calls regarding Chronic Care Management (CCM) with patients.

• Assist patients with chronic conditions between their appointments with healthcare providers.

• Provide education to patients about their health issues and care strategies.

• Collaborate with healthcare providers, pharmacies, caregivers, and community resources to coordinate care.

• Record patient interactions, update care plans, identify care gaps, and connect patients with existing resources.

• Dedicate the majority of your shift to making and receiving patient calls while delivering personalized support and care coordination.

⛳️ Requirements

• Active certification or registration as a Medical Assistant is required.

• Proficiency in both English and Spanish is preferred (for verbal and written communication).

• At least 1 year of experience as a Medical Assistant is necessary.

• Excellent skills in communication, documentation, and critical thinking are essential.

• Familiarity with EMR/EHR systems, Microsoft Office, and Google Workspace is preferred.

• Experience in care coordination, case management, call centers, triage, or geriatrics is an advantage.
