This is a fully remote position, open to applicants in United States.
• Act as the primary point of contact for cancer patients and their caregivers.
• Perform thorough clinical and needs assessments.
• Collaborate with oncologists, primary care physicians, survivorship specialists, genetic counselors, care advocates, and additional support team members.
• Deliver compassionate, empathetic care focused on patient advocacy.
• Address symptom management, care escalations, nutritional assistance, provider referrals, and planning for in-person care appointments.
• Communicate with patients via phone, asynchronous messaging, and video calls.
• Work alongside Care Advocates to address non-clinical support and care requirements.
• Assist in developing clinical protocols and best practices.
• Participate in care team meetings to monitor patient progress, provide updates, and collaborate on tailored support plans.
• Maintain detailed records of patient interactions.
• Contribute to the design of Color’s oncology clinic and enhance patient outcomes.
• Bachelor of Science in Nursing (BSN) is mandatory.
• Must hold an unrestricted active Registered Nurse (RN) license with the ability to obtain additional state licenses as necessary.
• An active Nurse Licensure Compact (NLC) multi-state license is required.
• A minimum of 7 years of nursing experience is needed, including at least 5 years in oncology healthcare environments.
• Outpatient experience is essential; inpatient experience is a bonus.
• An oncology-related certification is required: Oncology Certified Nurse (OCN), Advanced Oncology Certified Nurse (AOCN), Advanced Oncology Certified Nurse Specialist (AOCNS), or Certified Case Manager (CCM).
• Excellent organizational skills and the ability to manage multiple tasks in fast-paced settings.
• Strong verbal and written communication skills, with empathy and active listening capabilities.
• Proficiency with video conferencing, electronic health records, and telehealth tools.
• Medical proficiency in both English and Spanish is preferred.
• Must have the unrestricted right to work in the United States for any employer.
