This is a fully remote position, open to applicants in United States.
• Design and implement ongoing education initiatives focused on Oncology conditions and treatment strategies.
• Engage in and assist with departmental quality enhancement efforts and projects pertaining to Cancer.
• Serve as a resource for the clinical team on Oncology-related matters.
• Deliver education and follow-up care for newly diagnosed and established adults with Cancer.
• Perform telephonic patient assessments and create individualized care plans that encompass all aspects of Oncology self-management.
• Recognize and address patient concerns effectively.
• Collaborate with healthcare providers and vendors to ensure patient adherence to treatment plans.
• Identify and obtain resources to fulfill patient needs, including financial, psychosocial, physical, and clinical support.
• Cultivate relationships within the team to maintain a positive and supportive CareHarmony culture.
• Active Registered Nurse (RN) or higher license in a compact state is required.
• Clinical license must be current and in good standing.
• A minimum of five (5) years of professional experience in the field of Oncology is required.
• Oncology Clinical Nurse Specialist is preferred, but Oncology Certified Nurse (OCN) will also be considered.
• Experience in clinical training and process development is preferred.
• Familiarity with Epic systems.
• Bilingual candidates are encouraged to apply.
• Additional nursing licenses in single states are a plus.
