• Deliver thorough medication management that encompasses reconciliation, dose optimization, adherence support, and drug-level evaluations in accordance with evidence-based guidelines.
• Collaborate with physicians, specialists, and interdisciplinary teams to create personalized care plans under a Collaborative Practice Agreement (CPA) and facilitate smooth medication coordination.
• Assess the effectiveness and safety of therapies, identifying opportunities for optimization and intervening as necessary.
• Contribute to quality improvement and Risk Evaluation and Mitigation Strategies (REMS) initiatives.
• Offer recommendations to operations by assisting colleagues with complex prior authorizations, enhancing workflows, educating patients, and fostering continuous clinical development.
• Possess a Doctor of Pharmacy (PharmD) degree.
• Must hold a pharmacist license in Washington.
• Candidates must fulfill one of the following criteria:
• Completion of a PGY2 Oncology Residency.
• Completion of a PGY1 Ambulatory Care Residency along with a minimum of 3 years of experience as a clinical oncology pharmacist.
• A minimum of 3 years of experience as a clinical pharmacist in oncology, including at least 1 year practicing under a Collaborative Practice Agreement (CPA).
• Excellent communication, organizational, and technical skills.
