This is a fully remote position, open to applicants in Florida, +9 more states.
• Deliver diagnostic patient care primarily through virtual consultations via video conference or telephone.
• Act as the dispositional authority for clinical calls received after hours and on weekends.
• Evaluate, diagnose, treat, and document patients’ physical and psychosocial health conditions.
• Assess both acute and non-acute clinical issues through video conferencing or telephone consultations.
• Conduct and document physical examinations and patient medical histories.
• Analyze trends in patient conditions and create, document, and execute patient management strategies.
• Formulate care plans that include health education, physician referrals, case management referrals, and patient/family counseling.
• Gather and evaluate comprehensive and focused health data related to patients and their families.
• Collaborate with primary care providers and other medical professionals to report health information and ensure adherence to guidelines.
• Utilize telemedicine to achieve the best possible patient outcomes.
• Work with on-call primary care providers to enhance clinical results.
• Implement protocols and assess progress toward anticipated outcomes.
• Improve telehealth practices and systems by using data that shows program effectiveness.
• Communicate through various formats, tools, and technologies to provide care throughout the continuum.
• Offer safe, effective, financially responsible services along with exceptional customer service.
• Engage in clinical rounds, conferences, risk and quality management initiatives, and relevant meetings.
• Comply with departmental standards, regulatory requirements, quality care policies, and procedures.
• Operate under a written or electronic practice agreement.
• Contribute to the development of telehealth/telemedicine policies, procedures, and protocols.
• Initiate and engage in quality improvement efforts.
• Participate in committees that support organizational growth.
• Provide input on others’ practices to enhance patient care.
• Coordinate services with other programs.
• Travel to Georgia quarterly or Tennessee bi-annually for required onsite observations and medical record reviews when serving patients in those states.
• Execute additional duties as assigned.
• Bachelor’s degree in Nursing (BSN).
• Graduate of an Advanced Practice Nursing program.
• Master’s degree in Nursing.
• Board certification from AANP or ANCC.
• Basic Life Support (BLS) certification from the American Heart Association or American Red Cross upon hire.
• Current DEA number for Schedule II-V controlled substances may be necessary based on the state of practice.
• Multi-state licensure including Florida and at least three licenses among VA, GA, MI, MO, OH, PA, TN, TX, IL, and LA.
• A minimum of 4 years of acute/primary care clinical experience as a Nurse Practitioner.
• Preferred 2 years of experience in telehealth.
• Expert knowledge of essential job-related functions, practices, processes, procedures, techniques, and methods.
• Independent functioning as a certified practitioner with knowledge of medical practices and collaboration with physicians and other medical providers.
• Proven track record of meeting clinical performance metrics.
• Technical proficiency to conduct telemedicine visits in line with state and federal regulations.
• Excellent clinical judgment and problem-solving skills.
• Ability to prioritize tasks and perform under pressure.
• Capacity to provide constructive feedback.
• Professional communication and collaboration with physicians, patients, and team members.
• Ability to work effectively within a multidisciplinary team.
• Proficiency in Microsoft Office Suite, including Excel, Word, PowerPoint, and Outlook.
• Willingness to travel up to 10% for meetings and training sessions.
• Availability for evening, overnight, and/or weekend shifts based on the assigned schedule.
• Minimum requirement of working four holidays per calendar year.
• Spoken and written fluency in English.
• Willingness and ability to obtain independent/autonomous practice status as an NP in applicable states.
