Nurse Practitioner – Telehealth

This is a fully remote position, open to applicants in United States.

📋 Description

• Conduct annual risk assessments in a residential environment or via telehealth, within the Nurse Practitioner's scope of practice as assigned by the Collaborative Physician.

• Perform thorough in-home health risk evaluations to identify all existing and chronic health conditions, as well as to assess the physical, mental, and social needs present during the visit.

• Take patient history, perform examinations, and establish diagnoses.

• Provide written documentation of the patient visit in accordance with NCQA standards.

• Measure patient vital signs as required.

• Initiate case management referrals and coordinate with the primary care physician as needed.

• Liaise with patients, caregivers, agency nurses, other healthcare providers, and vendors as necessary to ensure accurate diagnoses.

• Carry out all clinical responsibilities while adhering to OSHA Universal Precautions.

• Uphold patient confidentiality at all times.

• Attend mandatory meetings and in-service trainings, and participate in committees upon request.

• Engage in professional development activities while maintaining professional licenses and affiliations.

⛳️ Requirements

• Possess an active/unrestricted nurse practitioner license to practice in the applicable states.

• Hold board certification from one of the following: American Nurses Credentialing Center (ANCC), American Association of Nurse Practitioners (AANP), or National Commission on Certification of Physician Assistants (NCCPA).

• Maintain an active BLS Certification.

• Reside in Indiana (IN).

• Currently enrolled in Medicare/Medicaid programs.

• Must have a valid driver’s license and a clean driving record.

• Exceptional electronic health record (EHR) skills.
