This is a fully remote position, open to applicants in Kentucky.
• Conduct annual risk assessments in residential environments or through telehealth, adhering to the Nurse Practitioner scope of practice.
• Execute thorough in-home health risk evaluations to uncover active and chronic health issues, as well as physical, mental, and social needs.
• Gather patient histories, perform evaluations, and establish diagnoses.
• Provide written documentation of patient visits in compliance with NCQA standards.
• Measure patient vital signs as required.
• Initiate case management referrals and liaise with Primary Care Physicians (PCPs) as necessary.
• Engage in communication with patients, caregivers, agency nurses, other healthcare providers, and vendors to ensure accurate diagnoses.
• Carry out clinical responsibilities while adhering to OSHA Universal Precautions.
• Uphold patient confidentiality at all times.
• Attend mandatory meetings and training sessions, and participate in committees when requested.
• Engage in professional development activities and maintain relevant professional licenses and affiliations.
• Possess an active/unrestricted nurse practitioner license for practice in coverage states.
• Achieve board certification from ANCC, AANP, or NCCPA.
• Hold an active BLS Certification.
• Maintain current enrollment in Medicare/Medicaid programs.
• Have a valid driver’s license with a clean driving record.
• Demonstrate exceptional EHR proficiency.
• Adapt to varying weather conditions and the unique environments of patients’ homes or residences.
• Exhibit full body motion capabilities, including the ability to handle and lift patients; possess manual and finger dexterity; maintain eye-hand coordination; have normal visual acuity and hearing; and perform standing, bending, walking, and stair climbing tasks.
• Ability to routinely lift and carry items weighing up to 50 pounds.
• Capability to travel by automobile or van up to 150 miles daily in both urban and rural settings.
• Willingness to drive when necessary.
• Geriatric training or experience is preferred.
• Strong skills in teamwork and the ability to maintain effective working relationships with patients, medical staff, and the public.
