Nurse Practitioner – Full Time

This is a fully remote position, open to applicants in Arizona, +2 more states.

📋 Description

• Perform virtual evaluations for patients suspected of having dementia, which includes initial cognitive assessments and ordering additional laboratory tests as necessary.

• Review medications for patients diagnosed with dementia, offering guidance to both patients and their healthcare teams to minimize the use of high-risk medications and initiate treatments that may alleviate symptoms where appropriate.

• Create detailed care plans for patients in collaboration with care navigators.

• Collaborate with care navigators and the patient’s primary care teams, including external healthcare providers and specialists, to ensure comprehensive support for the patient and their caregivers.

• Address other clinical needs that may arise, including urgent care or additional geriatric practice requirements as the practice develops.

⛳️ Requirements

• Family or adult-gerontology nurse practitioner with a minimum of 1 year of experience.

• Licensed in Arizona, Oregon, or Nevada with the capacity to obtain licensure for virtual/outpatient care in any state.

• Must not be barred from participating in any state or federal healthcare programs.

• A strong passion for providing care to patients with dementia.

• Willingness to learn and adapt to new technological systems.
