This is a fully remote position, open to applicants in Iowa, +2 more states.
• Delivers urgent healthcare services via telephone and tele-video channels to patients receiving home and community-based services through state Medicaid programs, dual-eligible members, and other memberships assigned by our MCO partners.
• Formulates and executes clinical care plans for adult patients dealing with chronic and complex health issues.
• Collects patient history, conducts physical examinations, and performs diagnostics as necessary, followed by the development and implementation of treatment plans aligned with the patient's healthcare objectives and current conditions.
• Identifies and addresses gaps in care.
• Addresses the physical and psychosocial needs of patients and their families with assistance and guidance from the company’s interdisciplinary team.
• Provides education to patients and families regarding medication administration, potential side effects, disease progression, dietary and nutritional guidance, medical adherence, and strategies for crisis anticipation and prevention.
• Maintains regular communication with other clinical team members and healthcare providers to ensure timely coordination of optimal care and resources for patients and their families, in accordance with state regulations and company health standards and policies.
• Keeps patient medical records and documentation compliant with state regulations and company policies and standards.
• Engages in continuing education as mandated by state requirements and certifying bodies.
• Prescribes medications as authorized by state prescribing regulations.
• A Master's degree in Nursing is required.
• An active national Nurse Practitioner (NP) certification is necessary.
• A valid, current, active, and unrestricted Family or Adult Nurse Practitioner (NP) license is required in the states of Texas, Iowa, or Kansas.
• A valid, current, active RN Compact license is needed.
• A minimum of 2 years of experience in managing complex care cases is required.
• Experience with Electronic Medical Records (EMR) is necessary.
• Bilingual or multilingual skills are required.
• Bilingual proficiency in Spanish is preferred.
• Possession of DEA registration or eligibility is preferred.
• An active Medicaid number is preferred.
• Experience in managing complex care cases for patients with developmental disabilities and chronic illnesses is strongly preferred.
