This is a fully remote position, open to applicants in Florida, +3 more states.
β’ Conduct preventative care screenings, perform medication reconciliation, diagnose medical conditions, and offer health coaching, referrals, and treatment when clinically appropriate.
β’ Reach out to members who are experiencing chronic health issues based on historical and suspected health data.
β’ Engage with members who are less likely to visit their primary care physician (PCP).
β’ Identify opportunities in the member's medical journey that can be enhanced by improving their access to care with a licensed provider.
β’ Gain cross-training in urgent care and/or virtual primary care service lines.
β’ Interact with members through virtual means, including assessment, diagnosis, and treatment.
β’ Ensure compliance with all relevant laws and regulations.
β’ Perform additional duties as assigned.
β’ PA or DNP, FNP, ENP from an accredited program.
β’ Board Certification (AANP, ANCC, or NCCPA).
β’ Compact RN (for NP only).
β’ Experience in Clinical Documentation Improvement and/or managing HEDIS quality measures.
β’ PA: Must be licensed in Georgia and one of the following states: Florida, Texas, or Ohio.
β’ NP: Must be licensed in at least two of the following states: Florida, Texas, Ohio, New Jersey.
β’ Bilingual - fluency in Spanish (reading, writing, and speaking).
β’ Ability to acquire licensure in additional states.
β’ A minimum of 3 years of experience in outpatient Family Medicine, Internal Medicine, or Primary Care.
