Nurse Practitioner, APRN

This is a fully remote position, open to applicants in Hawaii.

📋 Description

• Perform thorough Annual Wellness Visits (AWVs) for Medicare recipients utilizing a secure telehealth platform.

• Lead structured Advanced Care Planning (ACP) discussions to ensure patients' goals and preferences are well-documented.

• Evaluate and analyze patients’ medical histories, medications, functional and cognitive statuses, as well as their preventive care requirements.

• Identify and resolve care gaps, offering guidance on screenings, vaccinations, and chronic disease management strategies.

• Work collaboratively with interdisciplinary care teams and primary care providers to facilitate coordinated, patient-centric care.

• Keep precise and timely records in the electronic health record (EHR) system.

• Ensure adherence to CMS guidelines and clinical quality benchmarks.

⛳️ Requirements

• Possession of an active and unrestricted Nurse Practitioner license in Hawaii.

• At least 3 years of clinical practice experience, including Annual Wellness Visits and Advanced Care Planning.

• Strong understanding of Medicare regulations and preventive care protocols.

• Proficient and comfortable with virtual care technology and EHR systems.

• Exceptional communication and interpersonal abilities.

• Bilingual in Spanish is advantageous, though not mandatory.

• Capability to work autonomously and manage a flexible virtual schedule.
