This is a fully remote position, open to applicants in Virginia, +1 more state.
β’ Evaluate incoming patient referrals and determine their needs.
β’ Deliver suitable clinical assessments and facilitate connections to primary and specialty care providers.
β’ Educate patients on immediate care recommendations and preventive practices.
β’ Accompany patients to their appointments and manage travel, lodging, and itineraries.
β’ Evaluate and support multidisciplinary care coordination throughout the care continuum.
β’ Work collaboratively with care teams to streamline patient evaluations, treatments, survivorship, and end-of-life care.
β’ Organize and coordinate clinic appointments, virtual visits, and telehealth sessions.
β’ Relay clinical information between patients and healthcare providers.
β’ Assess health literacy and educational requirements; offer clinical education regarding diagnoses, treatments, side effects, and post-treatment care.
β’ Record patient needs, goals, preferences, and discussions in the medical documentation.
β’ Identify and resolve gaps, transitions, and obstacles to care.
β’ Communicate with referring and primary care practitioners.
β’ Provide leadership and support to Patient Navigators.
β’ Engage in service line team meetings.
β’ Valid unrestricted licensure with the WV Board of Registered Professional Nurses, or the appropriate state board where services will be provided, as a registered professional nurse.
β’ Current multi-state licensure through the enhanced Nurse Licensure Compact (eNLC) is acceptable.
β’ Minimum of three years of experience as a registered nurse.
β’ Capability to manage and uphold confidential information.
β’ Ability to thrive in a fast-paced and rapidly evolving environment.
β’ Exceptional verbal and written communication skills.
β’ Strong organizational and motivational abilities.
β’ Frequent activities including walking, standing, stooping, kneeling, reaching, pushing, pulling, lifting, and grasping may be required.
β’ Ability to remain seated for prolonged periods.
β’ Primarily scheduled from 7 AM to 7 PM, with rotations for 7 PM to 7 AM and weekends.
β’ Weekly scheduled hours: 36.
