Nurse, Multilingual – English, Mandarin, Cantonese

This is a fully remote position, open to applicants in Singapore.

📋 Description

• Reach out to caregivers before or during scheduled appointments to verify their arrival at the patient's residence.

• Ensure caregivers possess the most current and relevant patient Plan of Care.

• Send care plans and associated documentation to caregivers via email or secure transmission when necessary.

• Accurately document all communications, follow-ups, and care coordination activities in the EMR system.

• Report any patient care concerns, missed visits, or compliance issues to the relevant clinical supervisor.

• Uphold HIPAA compliance and maintain patient confidentiality at all times.

• Facilitate seamless continuity of care and prompt coordination between field staff and office personnel.

• Assist with basic care coordination and administrative nursing tasks as assigned.

⛳️ Requirements

• Registered Nurse with a valid New York License - USRN - NY.

• Proficiency in Cantonese and/or Mandarin as well as English is required.

• Previous experience in Home Health, Care Coordination, Case Management, or Remote Nursing is preferred.

• Strong verbal and written communication abilities.

• Familiarity with EMR/EHR systems.

• Excellent organizational and documentation skills.

• Capability to work independently in a remote setting.

• Proficient in email, phone communication, and basic computer systems.
