This is a fully remote position, open to applicants in Canada.
• Advocate for the health and wellbeing of First Nations individuals, families, and communities.
• Deliver culturally sensitive, relationship-focused community wellbeing nursing services informed by local knowledge and community needs.
• Provide health education on topics such as infection prevention and control, immunization, sexually transmitted and blood borne infections, mental wellness, nutrition, physical activity, and other health-related issues.
• Conduct needs assessments and assist in coordinated responses and community-led planning for population health.
• Administer immunizations according to Ontario's schedules, guidelines, and medical directives.
• Support initiatives related to cancer screening, maternal and child health, chronic disease management, STBBI, and tuberculosis.
• Carry out tuberculin skin tests and aid in contact tracing and outbreak management.
• Conduct home visits for wellness checks.
• Foster community relationships and engage in community events.
• Advocate for issues related to social determinants of health.
• Serve as a liaison among community-based workers, nurses, physicians, and visiting professionals.
• Connect communities with external resources, services, training programs, projects, and visiting professionals.
• Collaborate with Community Health Nurses, Health Directors, Community Health Representatives, mental health and addictions workers, school staff, and other departments.
• Offer training, clinical guidance, mentoring, and capacity-building to community-based workers.
• Manage sensitive and confidential information in line with organizational policies and PHIPA.
• Perform additional duties and provide further community support as needed.
• Active registration in good standing as a Registered Nurse (RN) or Registered Practical Nurse (RPN) with the College of Nurses of Ontario.
• A Registered Practical Nursing Diploma or Baccalaureate Degree in Nursing from an accredited college or university that includes community health nursing content.
• At least 1 year of prior work experience in community wellbeing (public health) or an equivalent combination of education and experience as approved by SLFNHA.
• Current BLS certification.
• A valid Ontario G class driver’s license and regular access to a vehicle.
• Knowledge of community wellbeing (public health) systems and First Nations health systems.
• Awareness of the historical and jurisdictional issues affecting First Nations health.
• Understanding of social determinants of health and contextual factors that influence the wellbeing of First Nations communities (public health).
• Familiarity with First Nations cultural values and norms.
• Experience working both independently and collaboratively in a team-oriented environment.
• Proficiency in computer and word processing skills, particularly with electronic medical record systems.
• Willingness to work irregular hours.
• Readiness to travel frequently to remote First Nations communities.
• Capacity to fulfill the role's requirements consistently.
• Ability to communicate in one of the First Nations dialects within the Sioux Lookout area is advantageous.
• Certification in vaccination programs is a plus.
