• Offer assistance to patients and healthcare providers by delivering information about Patient Support services, particular diseases, and therapies based on materials provided by the manufacturer and product monographs.
• Support patients and caregivers in performing assessments related to Patient Support services.
• Serve as a communication bridge between patients, families, and healthcare providers regarding medication access.
• Respond to general inquiries concerning program services, disease states, or product-related questions based on supplied materials.
• Provide ongoing support for disease states or product-related inquiries to promote patient adherence.
• Refer patients to their healthcare providers for clinical inquiries that extend beyond the program materials.
• Record patient information and interactions within the electronic patient record.
• Conduct regular file reviews and updates to accurately reflect the patient journey.
• Adhere to case management protocols, including data gathering, patient follow-up, monitoring therapy disruptions or discontinuations, adverse event reporting, and collaboration with primary healthcare providers.
• Work alongside the reimbursement specialist and physician to secure funding for patients' therapies and investigate alternative funding options.
• Ensure that patient services are organized and monitored according to KPIs, leading to expedited therapy access.
• Engage in specialized training sessions for specific therapies provided by the pharmaceutical team.
• Participate in pharmaceutical meetings, patient meetings, conferences, and collaborate with patient associations and advocacy groups as required.
• Coordinate with healthcare providers, pharmacies, and infusion/injection clinics to manage treatment logistics and resolve issues.
• Maintain open communication with the manager, team, and client regarding the management of patient cases.
• Assist in training new Nurse Case Managers and provide feedback on protocols, databases, processes, and inter-departmental procedures.
• Report Adverse Events / Severe Adverse Events in accordance with program procedures.
• A Diploma in healthcare or a related field is required; a Bachelor's degree is preferred.
• An active registration as a Licensed Nurse (RN or RPN/LPN) from the College of Nurses in the province of employment is mandatory.
• A minimum of two (2) to five (5) years of experience in healthcare, pharmaceuticals, insurance, or related fields is necessary.
• Ideal candidates will have a nursing background with experience in healthcare and case management within medical/hospital settings, insurance, or the biopharmaceutical industry.
• Proficiency in French and/or English may be necessary based on the program and geographical location.
