National Director of Patient Outcomes

This is a fully remote position, open to applicants in United States.

📋 Description

• Offer guidance, education, leadership, and mentorship for hospital case management departments and DPOs.

• Spearhead the development, implementation, and assessment of company-wide patient care initiatives related to case management.

• Create and oversee quality case management operational models that align with community needs and industry benchmarks.

• Formulate guidelines for collaboration with insurers, managed care organizations, referral providers, patients, and their families.

• Assess census, acuity, skill level, and staff availability to effectively manage staffing requirements and caseloads.

• Ensure compliance with state regulations, TJC, CMS, and other accreditation standards across facilities.

• Guarantee timely and accurate submissions of IRF-PAI documentation.

• Ensure FOM training is delivered to hospital personnel.

• Supervise chart review processes and ensure documentation accuracy throughout hospitals.

• Oversee the audit of the IRF-PAI final rule.

• Monitor and assess the Net Promoter Score (NRC) at each facility and provide necessary education.

• Engage in the preparation of annual capital and operating budgets and manage departmental expenditures.

• Adhere to approved safety standards and perform additional duties that promote organizational effectiveness.

⛳️ Requirements

• A minimum of five years of recent experience in an acute inpatient rehabilitation hospital setting is preferred.

• Current state clinical licensure is required.

• RN license is preferred.

• Must maintain an acceptable driving record, possess a current driver’s license, and be insurable.

• Understanding of clinical operations and procedures is essential.

• Familiarity with compliance and regulatory standards that govern health plans and patient care is necessary.

• Knowledge of treatment costs and financial support related to quality and efficiency is important.

• General computer proficiency, including data entry, word processing, email, and record management, is required.

• Strong critical thinking skills are essential.

• Ability to prioritize tasks, meet deadlines, and successfully complete complex assignments is necessary.

• Commitment to maintaining quality and safety standards is required.

• Must uphold confidentiality at all times.

• Willingness to travel between hospital locations is required.

• Ability to collaborate closely and professionally with colleagues at all organizational levels is essential.

• Effective organizational and time management skills are necessary.

• Exceptional verbal and written communication skills are required.

• Capability to meet the physical requirements of the position, including prolonged sitting, lifting/exerting up to 10 lbs, manual dexterity, close vision, and adjusting focus.
