This is a fully remote position, open to applicants in North Carolina.
• Manage the MFP Transition Coordination process for individuals moving from Skilled Nursing Facilities.
• Oversee the work of Transition Coordinators.
• Ensure the successful execution of transition services.
• Foster communication with internal teams, external partners, and community resources.
• A Bachelor’s degree in public service administration, psychology, sociology, social work, or a related human service program.
• Four years of experience in a directly related human services program, or in administrative or consultative roles within a human service program.
• Experience working with children, adolescents, and adults with Mental Health and Substance Use (MHSU) and Intellectual and Developmental Disabilities (IDD) diagnoses.
• Licensure/Certification Requirements: A valid NC driver’s license is required.
