Metabolic Health Nurse Practitioner, NP

This is a fully remote position, open to applicants in United States.

📋 Description

• Deliver direct patient care to Nourish patients, many of whom are dealing with metabolic disorders such as diabetes, obesity, dyslipidemia, hypertension, and fatty liver disease.

• Conduct telehealth appointments focusing on assessment, treatment planning, and ongoing management.

• Collaborate with registered dietitians and physicians to create comprehensive care plans that integrate medical therapy and lifestyle modifications.

• Manage medications and modify treatment protocols based on clinical data and patient responses.

• Order and interpret laboratory tests, monitor patient outcomes, and document encounters.

• Engage in continuous quality improvement, care coordination, and clinical innovation initiatives.

• Provide care to patients with the highest standards of personal and professional integrity and ethics, while maintaining a high level of confidentiality.

⛳️ Requirements

• Possess an active and unrestricted Nurse Practitioner license (multi-state licensure is a significant advantage).

• Board certification as a Family Nurse Practitioner (FNP), Adult-Gerontology Primary Care NP (AGPCNP), or a comparable qualification.

• Proven experience in metabolic, obesity, or diabetes medicine is highly preferred.

• At least 2 years of experience managing patients with metabolic conditions (e.g., diabetes, obesity, dyslipidemia, hypertension).

• Solid grounding in evidence-based lifestyle and medical management of chronic diseases.

• Exceptional communication, collaboration, and patient education abilities.

• Comfortable with telehealth service delivery and modern digital health technologies.
