This is a fully remote position, open to applicants in United States.
• Build effective and professional relationships with members while performing health risk assessments.
• Collaborate with the Integrated Care Coordination Team as required.
• Organize logistics to support care plan objectives and interventions.
• Address immediate concerns of members as necessary.
• Confirm member eligibility, enrollment history, demographics, and current health status prior to outreach.
• Conduct assessments by collecting information from members, families, providers, and other relevant stakeholders.
• Assist in the development and execution of care plans.
• Relay assessment information to the Care Coordinator when appropriate.
• Aid in health education, provide wellness materials, and offer coaching support.
• Ensure documentation aligns with Care Management protocols and guidelines.
• Enhance processes to improve member experiences, facilitate assessment completion, and ensure follow-up.
• Fulfill other responsibilities as assigned.
• A High School Diploma or General Education Diploma (GED) is mandatory.
• A minimum of two (2) years of experience in voluntary or paid work within community settings serving at-risk populations, focusing on service coordination, is preferred.
• Proficient in Microsoft Office applications, including Outlook, Word, and Excel.
• Sensitivity to and experience working with diverse cultures.
• Strong interpersonal skills.
• Capability to work both independently and collaboratively within a team environment.
• Ability to identify issues and opportunities, effectively communicating them to management.
• Growing knowledge of local, state, and federal healthcare regulations and company case management policies.
• Demonstrated compassion, support, and collaboration with members and their families.
• Self-motivated and curious.
• Comfort in posing relevant questions.
• Ability to thrive in a fast-paced workplace.
