Member Care Coordinator

This is a fully remote position, open to applicants in United States.

📋 Description

• Oversee the development of care plans tailored for pediatric patients with medical, behavioral, and social requirements.

• Engage daily with nurses, social workers, and healthcare providers to execute and evaluate care plans.

• Act as the main point of contact for families, aiding in scheduling, referrals, and follow-up activities.

• Monitor patient progress, document interventions, and ensure accurate record-keeping within the EMR and Care Management System.

• Promote effective communication between internal teams and external healthcare providers, specialists, and community resources.

• Recognize care gaps and escalate issues to clinical staff as necessary.

• Assist with transitions of care, such as hospital discharges and specialty referrals.

• Ensure adherence to care management protocols and uphold quality standards.

⛳️ Requirements

• Proven experience in care coordination, case management, healthcare, or a closely related field.

• Exceptional organizational, communication, and problem-solving abilities.

• Capability to juggle multiple patients and priorities in a dynamic environment.

• Comfortable working autonomously in a remote setting while fostering strong team collaboration.

• A minimum of 3 years of relevant experience in care coordination, case management, or a similar healthcare position (pediatrics is highly preferred).

• Demonstrated history of collaboration with nurses and/or social workers in a clinical or care management context.

• Solid understanding of healthcare systems, referrals, and community resources.

• Proficient in using electronic medical records (EMR) systems.
