Medical Social Worker

This is a fully remote position, open to applicants in California.

📋 Description

• Oversee the daily operations of the social services team, ensuring adherence to policies, procedures, and effective care coordination with other clinical providers.

• Recruit, onboard, and mentor social workers, offering clinical supervision, observing their practices, and facilitating their professional development.

• Manage the assessment, planning, and evaluation processes for social services care.

• Ensure that clinical documentation complies with state, federal, and ACHC standards.

• Collaborate with the Executive Director on the annual budget and resource allocation for the department.

• Enhance knowledge of community resources and connect patients and referral sources with appropriate support services.

• Organize in-service training and continuing education programs.

• Contribute to quality improvement efforts and identify opportunities to enhance patient care.

• Perform additional duties as necessary to support patient services.

⛳️ Requirements

• Master of Social Work (MSW) from an accredited institution.

• Valid California license as a Licensed Clinical Social Worker (LCSW).

• Minimum of 3 years of management experience, ideally in home care settings.

• Comprehensive understanding of healthcare regulations, ACHC standards, and business operations.

• Exceptional communication skills with the ability to establish trust among patients, families, staff, and the community.

• Self-motivated, organized, and adaptable; proactive and capable of collaborating effectively with others.
