
Medical Assistant – Clinical Documentation Support

This is a fully remote position, open to applicants in Pennsylvania.
• Assist in the preparation of provider-facing documentation for PACE Participants by arranging existing medical records and other authorized clinical information.
• Collect and organize existing medical records and approved clinical documentation for provider evaluation.
• Create provider-facing documentation packets and templates prior to scheduled Participant appointments.
• Examine assigned work queues and obtain approved reports and supporting documentation.
• Structure hospital, emergency department, skilled nursing facility, specialist, laboratory, imaging, medication, vital sign, assessment, and care plan details.
• Ensure that documentation materials are complete, accurate, well-organized, and accessible within specified timelines.
• Monitor missing records and follow up through designated channels.
• Keep precise work queues, preparation trackers, and provider visit readiness logs.
• Track documentation status and direct items to the appropriate provider or CDI Nurse.
• Collaborate effectively with providers, Medical Records, Clinical Operations, CDI Nurses, scheduling, and other support teams.
• Report unclear, inconsistent, complex, or audit-sensitive information to the relevant clinical leader.
• Utilize approved templates and impartial language when developing documentation support materials.
• Assist with follow-up tracking after provider appointments.
• Engage in quality improvement, workflow development, and training activities.
• Uphold Participant confidentiality and adhere to HIPAA and organizational policies.
• Perform additional duties as assigned.
• High School Diploma or GED is required.
• Prior medical coding experience with a strong focus on CC is required.
• Experience in reviewing provider charts and understanding provider documentation practices is required.
• Familiarity with electronic medical records is required.
• Must be organized, reliable, and highly detail-oriented.
• Strong knowledge of medical coding with a CC-focused approach.
• Understands provider charting and the appropriate documentation of clinical conditions in medical records.
• Comfortable reviewing provider documentation and identifying any missing or incomplete information for proper assessment.
• Comfortable handling confidential healthcare information.
• Ability to manage multiple priorities, deadlines, and work queues.
• Excellent written and verbal communication skills.
• Capable of working independently while also collaborating with a broader clinical team.
• Comfortable utilizing electronic medical records and learning new technologies.
• Able to identify when information needs to be escalated to a provider, CDI Nurse, or clinical leader.
• Recognizes the significance of maintaining neutral, factual documentation.
• Proficient in computer skills, including Microsoft Office, Outlook, Excel, and Teams.
• CPR and First Aid Certification (or willingness to obtain post-hire; training will be provided!).
• Medical Assistant training, diploma, certificate, or equivalent healthcare administrative or clinical support experience is preferred.
• Experience in medical records, healthcare administration, clinical documentation, or provider support is preferred.
• Experience working with the frail and elderly population, PACE, geriatrics, or value-based care is advantageous.
• Reliable availability and connectivity during scheduled work hours.
