This is a fully remote position, open to applicants in California.
• Act as a liaison between clinicians and patients, ensuring a smooth, efficient, and patient-centered care experience.
• Assist patients with the intake processes, establishing goals, tracking progress, managing their appointments, and obtaining labs/prescriptions as necessary.
• Conduct a substantial number of telehealth intake appointments (via phone/video) to evaluate patient goals, complete clinical screenings, and clarify service offerings and coverage.
• Oversee the complete telehealth scheduling process, providing both technical and logistical support. Manage lab orders, prescription logistics, and maintain accurate EHR profiles.
• Review provider clinical notes to ensure that patients comprehend and can access their customized care plans. Perform strategic outreach (calls, texts, emails) to facilitate care initiation, monitor behavioral goals, and assist patients in overcoming setbacks or motivational challenges.
• Act as the main problem solver for patient-related barriers (including technical issues, insurance inquiries, and care hesitancy), escalating concerns as necessary to guarantee uninterrupted communication.
• Collaborate with clinical teams, provider operations, patient support, and product departments to enhance patient flow and improve the overall care experience.
• Engage in process improvement initiatives and cross-functional feedback loops to refine and elevate the care delivery model.
• A current and valid National Medical Assistant Certification (CCMA, CMA, RMA, or NCMA) is required.
• A minimum of 3 years of experience in direct patient care, care coordination, digital health, or a related health/human services field.
• Outstanding verbal and written communication skills, with a demonstrated ability to build trust and express empathy.
• Proven history of meeting or surpassing measurable KPIs (e.g., patient engagement, satisfaction, conversion, and retention).
• Capability to efficiently manage a high volume of patient interactions without compromising quality or attention to detail.
• A solution-focused mindset with the flexibility to thrive in a fast-paced, changing healthcare environment.
• Proficiency in EHR/CRM platforms, scheduling systems, telehealth software, and remote collaboration tools.
