This is a fully remote position, open to applicants in United States.
• Establish trustworthy relationships with patients, their families, caregivers, and healthcare providers.
• Engage with high-risk and targeted patient groups via phone calls, text messages, video consultations, online portals, emails, and AI agents.
• Evaluate medical, behavioral, social, emotional, and financial needs of patients.
• Educate patients about care plans, health objectives, and preventive healthcare using motivational interviewing techniques.
• Perform nonclinical assessments, surveys, and interventions.
• Identify and escalate changes in patient conditions, adherence risks, and clinical issues.
• Prioritize interventions based on clinical insights and support population health initiatives.
• Coordinate referrals, transportation, appointments, specialty care, and community resources.
• Escalate medical concerns to RN Care Managers, Social Workers, provider offices, or management.
• Assist with eCQM, HEDIS, and commercial payer quality measures.
• Support risk adjustment and ensure accurate documentation of patient conditions.
• Address care gaps and minimize unnecessary emergency visits, hospitalizations, and readmissions.
• Record interactions, assessments, and interventions in EHR and Guidehealth systems.
• Manage referrals, track tasks, complete reconciliations, and maintain thorough records.
• Contribute to the design and implementation of AI outreach for identified populations.
• Must be fully bilingual in Arabic and English, including comprehensive medical terminology.
• A minimum of 2–3 years of relevant experience in patient navigation, medical assistance, health promotion, care coordination, or similar roles.
• Must possess a valid Medical Assistant Certification.
• Experience working directly with patients in both clinical and non-clinical environments, preferably with complex medical or social needs.
• Proven experience with EHR documentation and effectiveness in remote work settings.
• Proficiency with EHR systems such as Epic, Athena, and Cerner.
• Familiarity with analytics tools and Microsoft Office applications.
• Ability to adhere to Guidehealth’s policies and procedures.
• Capability to safeguard PHI, PII, and Guidehealth intellectual property.
• Reliable internet connection suitable for video calls and internal/external systems, with a minimum of 100 Mbps download and 10 Mbps upload speed.
• Availability during designated working hours and readiness to participate in virtual meetings with cameras on.
