This is a fully remote position, open to applicants in United States.
• Perform outbound phone calls to patients to provide individualized coaching and support for those managing chronic conditions.
• Assist patients in implementing tailored care plans and reaching their objectives through education and coaching.
• Carry out thorough evaluations of patients' health status and their readiness for change.
• Inform patients about disease management, adherence to medication, and preventive measures.
• Track patients' progress, compliance with treatment plans, and overall health outcomes.
• Record patient interactions and identify obstacles to care.
• Collaborate in real-time with healthcare providers and patient support teams.
• A valid Medical Assistant certification from an accredited organization is mandatory.
• A minimum of 5 years of experience as a Medical Assistant is required.
• Capability to work remotely (including having reliable high-speed WiFi) is essential.
• Experience in internal medicine, cardiology, nephrology, or with the geriatric population is preferred.
• Prior experience in providing education and support to patients managing chronic conditions is preferred.
• Proficiency in Spanish is highly preferred.
• Familiarity with AI-assisted clinical tools is required.
