This is a fully remote position, open to applicants in United States.
• Assist clients in navigating the complexities of healthcare, elder care, and community support systems.
• Provide education and guidance on healthcare services, benefits, care options, and available resources.
• Aid clients and their family members in understanding healthcare processes and advocating for their own care needs.
• Conduct research to identify suitable resources, referrals, and support services tailored to individual client circumstances.
• Maintain precise documentation of client interactions and recommendations for resources.
• Communicate effectively with clients, healthcare providers, community agencies, and other stakeholders while ensuring adherence to confidentiality requirements.
• Collaborate with internal teams and external organizations as necessary to enhance client outcomes.
• Bachelor’s degree in Social Work (BSW) or at least five years of relevant experience in hospital systems, hospice care, geriatric care management, or a closely related field.
• Strong comprehension of healthcare systems and care coordination processes.
• Familiarity with HIPAA regulations, third-party payer systems, CPT codes, and healthcare documentation requirements.
• Exceptional verbal communication, active listening, and client engagement skills.
• Strong organizational and time-management capabilities.
• Proficient in remote communication technologies and standard office software applications.
