📋 Description

• Lead person-centered planning meetings to organize care and services.

• Identify, coordinate, and assist participants in accessing essential LTSS services and other Covered Services, along with non-covered medical, social, housing, educational, and additional services and supports.

• Aid in the completion of 1915 (I) assessments based on individual member needs.

• Educate participants about the available LTSS required needs assessments, the care plan process, service alternatives, and delivery options.

• Provide guidance and support to members regarding their rights and responsibilities, as well as the grievance process.

• Conduct Discovery activities (information gathering and assessment) as part of the planning process.

• Assist members or their legally responsible persons in selecting service providers while ensuring objectivity.


⛳️ Requirements

• Must possess QP status.

• Fully licensed by the NC governing board overseeing Human Services professions; OR

• Fully licensed Registered Nurse authorized to practice in NC by the NC Board of Nursing, with four (4) years of full-time experience serving the MH/DD/SA population.

• A graduate with a Master’s degree in a human service field, along with one year of full-time, pre or post-graduate degree experience in MH/DD/SA.

• A graduate with a bachelor’s degree in a human service field and two years of full-time, pre or post-bachelor's degree experience in MH/DD/SA.

• A graduate with a bachelor’s degree in a field outside of human services, possessing four years of full-time, pre or post-bachelor's degree experience in MH/DD/SA.

• Care manager must fulfill the minimum QP requirements and have at least two (2) years of prior experience in LTSS and/or HCBS coordination, care delivery monitoring, and care management.

• Must hold a valid driver’s license.
