LPN, Virtual Support

This is a fully remote position, open to applicants in Kentucky.

📋 Description

• Evaluate, prioritize, and oversee incoming Epic InBasket messages

• Assess patient advice requests utilizing established clinical protocols

• Manage prescription refill requests in accordance with BHMG policies and procedures

• Analyze laboratory and diagnostic results for suitable routing and follow-up

• Refer complex or urgent clinical issues to VASER providers or primary care teams

• Recognize urgent patient needs that necessitate same-day appointments

• Address patient messages via MyChart and telephone communications

• Inform patients about medications, treatment plans, and provider recommendations

• Arrange follow-up appointments when clinically necessary

• Reinforce care plans and motivate patient participation

• Assist primary care teams throughout the health system and contribute to the transformation of ambulatory care delivery

• Collaborate with physicians, Advanced Practice Providers (APPs), pharmacists, and practice teams

⛳️ Requirements

• Completion of an accredited LPN program

• Valid LPN license for Kentucky or Indiana

• At least two years of experience in ambulatory or primary care settings

• Proficiency with Epic electronic medical records

• Exceptional written and verbal communication abilities

• Strong clinical assessment and critical thinking capabilities

• Capacity to independently prioritize a high volume of asynchronous patient care tasks

• Ability to work collaboratively with physicians, APPs, pharmacists, and practice teams
