This is a fully remote position, open to applicants in Michigan.
β’ Conduct phone consultations with patients participating in care management and/or remote patient monitoring programs.
β’ Offer support and education regarding patients' chronic illnesses.
β’ Inform patients about their health conditions and empower them with strategies for lifestyle and behavior modifications.
β’ Assist patients in establishing and achieving goals in line with provider-approved care plans.
β’ Ensure patient records are accurate and current, documenting interactions and care plans according to protocols.
β’ Address patient concerns and barriers to care, finding practical solutions to enhance adherence and outcomes.
β’ Communicate effectively and compassionately with patients.
β’ Follow established workflows for conveying patient needs to providers.
β’ Engage in training sessions, team meetings, and initiatives for quality improvement.
β’ Respond to remotely transmitted patient data, including blood pressure, blood glucose, weight, and pulse oximetry, in accordance with approved partner workflows.
β’ Collaborate with primary or specialty physician practices, healthcare systems, and patients' care teams.
β’ Manage an assigned group of patients throughout the month and assist with additional patient-related tasks as needed.
β’ Must possess a current, valid, and in good standing Michigan Nursing License (LPN).
β’ Minimum of 3 years of experience in primary care, cardiology, internal medicine, home care, or chronic care management/remote patient monitoring.
β’ Strong communication and interpersonal skills are essential.
β’ Excellent organizational and time management skills are required.
β’ Proficiency in electronic health records (EHR) and care management software is necessary.
β’ Ability to work independently as well as collaboratively within a team.
β’ Empathy and a patient-centered approach to care are crucial.
β’ A reliable internet connection is required.
β’ Must have a HIPAA-compliant work area.
β’ Proficiency with virtual communication tools (e.g., Zoom, Slack) is preferred.
β’ Compact Nursing License is a plus.
β’ Candidates must be based in Michigan or be remote within the United States.
