This is a fully remote position, open to applicants in United States.
• Develop personalized care plans for patients and deliver care management and coordination services utilizing Ennoble Care’s electronic medical record system.
• Maintain regular communication with patients to facilitate care coordination, provide support, and ensure adherence to care management programs aimed at improving outcomes.
• Accurately document all client interactions, whether verbal or written.
• Communicate with the Primary Care Clinician or their designee regarding urgent patient needs and/or critical incidents to ensure that comprehensive care plans are accurate and complete.
• Keep the Team Supervisor updated on all matters relevant to the care plan process and any known or perceived challenges.
• Exhibit the ability to collaborate with diverse organizational areas to meet the needs of Ennoble Care’s patients, their families, and partner facilities.
• Acquire proficiency in using technology tools including secure email, telephone systems, and electronic medical records.
• Adhere to documentation protocols and best practices for daily work logs, escalation of client issues, and internal communication.
• Display excellent customer service skills as evidenced by positive feedback from clients and patients.
• Actively contribute as a supportive team member by fostering a productive and constructive environment for all department members.
• Must possess a Licensed Practical Nurse (LPN) qualification.
• Should be comfortable speaking on the phone for extended periods throughout the day.
• Must exhibit compassion and empathy towards patients, consistently demonstrating outstanding customer service.
• Ability to take precise notes and document each task promptly.
• Capacity to multitask across various patients and workstreams while remaining organized and efficient with time management.
• Ability to thrive in a dynamic, fast-paced environment.
• Availability to work Saturday and Sunday from 8:00am to 4:30pm (EST), as well as Monday, Tuesday, and Wednesday from 9:30am to 6:00pm (EST).
• Proficiency in computer usage, including Outlook and other Microsoft Office applications.
• Familiarity with basic healthcare terminology, conditions, roles, and fundamental care principles is advantageous.
• Candidates must successfully pass a drug screening and background check, possess a positive attitude, adapt positively to change, be a team player, and be willing to continuously learn new skills.
