This is a fully remote position, open to applicants in California.
• Oversee incoming calls from patients and facilities, ensuring timely and professional responses.
• Accurately document interactions in the electronic health record (EHR).
• Address voicemail, electronic, and fax communications in accordance with program turnaround standards.
• Elevate urgent clinical issues to providers following established protocols.
• Act as a liaison among patients, providers, skilled nursing facilities (SNFs), hospitals, and community partners.
• Assist patients in navigating healthcare services and accessing necessary resources.
• Support follow-up on orders, care plans, and recommendations from providers.
• Identify barriers to care and encourage patient self-management.
• Monitor quality and program metrics using internal tools and dashboards.
• Help enhance workflows to facilitate care transitions and minimize gaps in care.
• Engage in quality improvement and patient satisfaction initiatives.
• Take part in interdisciplinary meetings and case discussions.
• Uphold HIPAA compliance and regulatory documentation standards.
• Foster professional relationships with both internal teams and external partners.
• A minimum of 2 years of experience in healthcare, care coordination, or patient support.
• Experience in SNF, post-acute, managed care, or value-based care is preferred.
• Excellent communication and documentation skills.
• Capability to manage multiple priorities in a dynamic environment.
• A Bachelor’s degree is preferred.
• Bilingual proficiency in Spanish is preferred.
