LPN Escalation

This is a fully remote position, open to applicants in United States.

📋 Description

• Act as an escalation point for both outbound and inbound teams, as well as enrollment teams.

• Quickly assess patient history, vital signs, and RPM readings to evaluate urgency.

• Engage in direct outreach to patients regarding escalated issues.

• Oversee RPM alerts and readings that fall outside of the normal range.

• Elevate issues needing RN or provider-level evaluation to the RN Escalation team.

• Liaise with the offices of ordering providers to communicate patient concerns.

• Thoroughly and compliantly document all escalation activities.

• Uphold productivity and quality standards while navigating varying volumes of escalated cases.

• Must possess a current active, compact LPN/LVN license.

• Demonstrated strong triage and critical-thinking abilities.

• Capability to work autonomously and apply sound clinical judgment within the LPN scope of practice.

• Proven excellence in adhering to established clinical escalation protocols and procedures.

• Ability to remain composed and focused in high-pressure situations.

• Strong communication skills for effective coordination with providers' offices and the RN Escalation team.

• Willingness to support colleagues.
