LPN, Care Navigator

This is a fully remote position, open to applicants in United States.

📋 Description

• Conduct phone consultations with patients participating in care management and/or remote patient monitoring programs.

• Offer support and education regarding patients’ chronic conditions.

• Inform patients about health issues and empower them with effective lifestyle and behavioral strategies.

• Aid patients in establishing and achieving goals that align with provider-approved care plans.

• Keep patient records accurate and current.

• Document patient interactions and care plans in accordance with protocols.

• Address patient concerns and obstacles to care while identifying practical solutions.

• Relay patient needs to providers following established workflows.

• Engage in training sessions, team meetings, and quality improvement efforts.

• Respond to remotely sent patient data, including blood pressure, blood glucose, weight, and pulse oximetry, as per approved partner workflows.

• Build relationships with assigned patients to foster chronic illness management and enhance health outcomes.

⛳️ Requirements

• Must possess a current, valid, and in good standing Multistate/Compact Nursing License (LPN/LVN).

• Additional state licenses may be necessary.

• A minimum of 3 years of experience in primary care practice, cardiology, internal medicine, home care, or chronic care management/remote patient monitoring.

• Strong communication and interpersonal abilities.

• Excellent organizational skills and time management capabilities.

• Proficiency in utilizing electronic health records (EHR) and care management software.

• Capacity to work both independently and collaboratively within a team.

• Demonstrated empathy and a patient-centered focus in care delivery.

• A reliable internet connection is essential.

• A HIPAA-compliant workspace is required.

• Familiarity with virtual communication platforms such as Zoom and Slack.
