Long Term Care Coordinator – Float Team

This is a fully remote position, open to applicants in Connecticut, +2 more states.

📋 Description

• Handle incoming calls from members regarding questions or issues related to benefits, care coordination, program inquiries, connections to care managers, and screening assessments.

• Accurately document calls in real-time within the designated PHI system.

• Oversee an assigned caseload of members in need of short-term care coordination and health navigation.

• Collaborate with the interdisciplinary care team to swiftly communicate any member issues or needs.

• Effectively monitor member screening to enhance quality and cost outcomes.

• Conduct outreach calls to verify that services are being provided.

• Undertake additional responsibilities as assigned.

⛳️ Requirements

• Certification as a Medical Assistant (CMA), Certified Nursing Assistant (CNA), Registered Medical Assistant (RMA), or relevant medical training or work experience.

• Possession of a high school diploma or GED.

• Work experience that necessitates strong verbal and written communication skills.

• Proficient in grammar, spelling, and punctuation.

• Ability to thrive in a fast-paced environment while managing multiple priorities simultaneously.

• Preferred: Associate’s Degree.

• Preferred: prior experience in a call center, care management, or healthcare setting.

• Preferred: familiarity with medical terminology, Medicaid, and Child Health Plus benefit products.

• Excellent telephone, organizational, and customer service abilities.
