📋 Description

• Instructing individuals in new skills according to the Individualized Service Plan (ISP)

• Facilitating educational activities in a home environment

• Adhering to the participant's care plan to ensure that care needs and Activities of Daily Living (ADLs) are addressed as specified

• Ensuring opportunities for community engagement and social interaction
