Licensed Vocational Nurse, Remote Days 7a-3:30p PST

This is a fully remote position, open to applicants in United States.

📋 Description

• Serve as a telehealth-enabled care coordinator and case management extender within the Advanced Care at Home initiative.

• Function under the supervision of an RN/provider and adhere to telehealth regulations.

• Facilitate patient throughput, care coordination, and ongoing case management activities throughout the acute care episode provided in the home setting.

• Combine real-time workflow execution with case management support.

• Ensure that patients receive timely, appropriate, and coordinated services that align with clinical plans, regulatory standards, and program protocols.

• Operate under the guidance of an RN/provider and within the scope of LVN licensure.

• Oversee daily care activities for a virtually managed patient population.

• Carry out additional responsibilities as assigned.

⛳️ Requirements

• Current LVN/LPN license in good standing; California license is preferred.

• A minimum of 2 years of clinical experience (acute care, home health, ambulatory, or case management).

• Proficiency with EMR/EHR systems and clinical workflows.

• Excellent telephonic and virtual communication skills.

• Capacity to manage multiple priorities in a dynamic, remote environment.

• Proven ability to operate within scope and escalate issues as needed.

• This position may be performed remotely from anywhere in the United States, with the exception that it may not be filled or conducted in Washington, California, New York, Illinois, Montana, Pennsylvania, Nevada, or Colorado.
