Licensed Vocational Nurse, Remote 3p-11:30p PST

This is a fully remote position, open to applicants in United States.

📋 Description

• Serve as a telehealth-enabled care coordinator and extend case management within the Advanced Care at Home program.

• Function under the supervision of RN/providers while adhering to telehealth regulations.

• Facilitate patient throughput, care coordination, and ongoing case management throughout the acute care episode provided at home.

• Merge real-time workflow execution with case management support.

• Ensure that patients obtain timely, suitable, and coordinated services that align with clinical plans, regulatory standards, and program protocols.

• Operate under the guidance of an RN/provider within the scope of LVN licensure.

• Organize daily care activities for a virtually managed patient population.

• Appropriately escalate issues and undertake additional responsibilities as assigned.

⛳️ Requirements

• Current LVN/LPN license in good standing.

• California License is preferred.

• A minimum of 2 years of clinical experience in acute care, home health, ambulatory care, or case management.

• Familiarity with EMR/EHR systems and clinical workflows.

• Excellent telephonic and virtual communication abilities.

• Capacity to handle multiple priorities in a fast-paced, remote setting.

• Proven ability to work within scope and escalate matters appropriately.

• This position can be performed remotely from any location in the United States, with the exception that it cannot be filled or executed in Washington, California, New York, Illinois, Montana, Pennsylvania, Nevada, or Colorado.
