This is a fully remote position, open to applicants in Tennessee.
• Evaluates, plans, and implements personalized care strategies tailored to each member, focusing on the most suitable and least restrictive care levels.
• Works collaboratively with members, their families, and healthcare professionals to create a customized care plan.
• Facilitates individual counseling sessions and group therapy for both adolescents and adults.
• Identifies and initiates referrals for various social service programs, including financial, psycho-social, community, and state-supported services.
• Acts as an advocate for members and families to ensure that the patient’s needs and preferences are adequately represented and supported by the healthcare team.
• Applies approved clinical criteria to evaluate and determine the appropriate care level for members.
• Records all member assessments, care plans, and the referrals made.
• Accountable for meeting established goals and Key Performance Indicators (KPIs).
• Engages in learning about the Substance Use Disorder Program model and takes responsibility for outcome-based care.
• Participates in interdisciplinary team meetings and utilization management rounds, providing relevant information to facilitate safe transitions of care.
• Encourages responsible and ethical management of company resources.
• Upholds excellent punctuality and attendance throughout work hours.
• Must hold an LCSW, LMFT, or LPC license in Tennessee.
• Prior experience with the substance abuse population is preferred.
• Background in maternal and substance abuse is advantageous.
• Experience working with both adults and adolescents.
• Knowledge of Medicare and Medicaid procedures is beneficial.
• Experience with remote work is preferred.
