Therapy Continuity Coordinator

This is a fully remote position, open to applicants in United States.

📋 Description

• This position is a non-clinical, operational role that collaborates closely with Sleep Technologists during the transition from initial treatment setup to long-term resupply.

• Once patients move into the resupply phase, you will oversee follow-up, execution, and documentation to guarantee a seamless and dependable experience.

• Operate within a tech-enabled, AI-assisted care environment.

• Engage with clinical, billing, and operations teams.

• Assist in tracking engagement and reorder metrics while contributing to a scalable operational framework within a rapidly expanding digital sleep clinic.

• Conduct timely and proactive resupply outreach.

• Adhere to structured retention workflows.

• Keep accurate records of reorder tracking.

• Communicate with patients in a clear and compassionate manner.

• Appropriately escalate clinical or billing concerns.

• Support adherence monitoring initiatives.

⛳️ Requirements

• Minimum of 2 years in healthcare coordination, DME, or care navigation.

• Experience with PAP/CPAP resupply is a strong advantage.

• Excellent patient communication abilities with a high degree of empathy and professionalism.

• Capability to work independently in a fast-paced, remote environment while effectively collaborating with cross-functional teams.

• Proficient in navigating digital tools and care platforms.

• Data-driven mindset with the capacity to track tasks, maintain precise records, and follow structured workflows.

• Comfortable managing proactive outreach and patient follow-up while paying close attention to detail.

• Genuine dedication to enhancing patient experience, therapy adherence, and continuity of care.

• An operational and automation-focused mindset.

• Availability to work during Central Time (CST) business hours is preferred.
