Licensed Practical/Vocational Nurse

This is a fully remote position, open to applicants in California.

📋 Description

• Deliver direct patient care in line with the state Nurse Practice Act.

• Execute and adhere to personalized care plans developed by the Registered Nurse (RN).

• Educate patients and their families on self-care methods and information related to diagnoses.

• Accurately document care provided in a timely manner.

• Work collaboratively with the care team, which includes Occupational Therapy (OT), Physical Therapy (PT), Home Health Aides (HHA), and other disciplines, while communicating patient needs to the RN or Clinical Supervisor.

• Utilize supplies and equipment safely, ensuring adherence to aseptic protocols.

• Perform additional duties as necessary to support patient services.

⛳️ Requirements

• Completion of an accredited practical or vocational nursing program.

• Active Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) license in California.

• A minimum of 3 years of nursing experience, preferably in home health or medical/surgical settings.

• Current CPR certification from the American Heart Association (AHA).

• Valid driver's license, reliable insured vehicle, and capability to travel to patient residences.

• Excellent communication skills along with a collaborative, patient-centered approach.
