Licensed Practical/Vocational Nurse

This is a fully remote position, open to applicants in California.

📋 Description

• Deliver direct patient care in compliance with the state Nurse Practice Act.

• Execute and adhere to personalized care plans created by the RN.

• Educate patients and their families on self-care strategies and relevant diagnostic information.

• Accurately document care in a timely manner.

• Collaborate with the care team, including OT, PT, HHA, and various other disciplines, while communicating patient needs to the RN or Clinical Supervisor.

• Utilize supplies and equipment safely, following aseptic techniques.

• Perform additional duties as required for patient services.

⛳️ Requirements

• Graduate from an accredited practical or vocational nursing program.

• Hold an active LPN or LVN license in California.

• Possess 3+ years of nursing experience; background in home health or med/surg preferred.

• Maintain current CPR certification from AHA.

• Have a valid driver’s license, a reliable insured vehicle, and the ability to travel to patient homes.

• Exhibit strong communication skills and a collaborative, patient-first approach.
