This is a fully remote position, open to applicants in United States.
• Perform outbound telephonic outreach to patients and offer individualized coaching and support for those managing chronic illnesses, including but not limited to type 2 diabetes, hypertension, and cardiovascular disease.
• Assist patients in implementing tailored care plans and reaching their objectives by delivering education and coaching centered on behavior modification, nutrition, physical activity, and self-management strategies, utilizing evidence-based methods such as motivational interviewing and SMART goals.
• Carry out thorough assessments of patients' health status, lifestyle choices, nutritional habits, and their readiness to make changes.
• Inform patients about disease management, medication adherence, symptom recognition, and preventive strategies, while performing care delivery tasks in line with Cadence clinical policies and established protocols, under suitable clinical supervision.
• Track patients' progress, adherence to treatment plans, and health outcomes through regular check-ins and remote monitoring tools.
• Record patient interactions, outcomes, and follow-up plans in the Cadence platform; identify barriers to care (including social needs) and assist patients in connecting with resources or internal support networks.
• Collaborate in real-time with clinicians and patient support teams using tools such as our Cadence Platform, Google Workspace, and Slack, and provide feedback to enhance workflows in a dynamic, change-oriented environment.
• A valid compact multi-state LPN/LVN license in the state of residence is mandatory.
• A minimum of 5 years of clinical experience as a Licensed Practical Nurse is essential.
• Readiness for remote work (including reliable high-speed WiFi) is required.
• A current CA state LVN license is necessary.
• Proficiency in Spanish is preferred.
• Experience in providing patient education and support for individuals managing chronic conditions is preferred.
• Familiarity with AI-assisted clinical tools (like Gemini, Open Evidence, or similar platforms) and the capability to communicate credibly with providers and patients regarding where AI supports care delivery and where human judgment remains essential.
