Licensed Practical Nurse

This is a fully remote position, open to applicants in California.

📋 Description

• Deliver direct patient care in compliance with the state Nurse Practice Act.

• Execute and adhere to personalized care plans set forth by the RN.

• Teach patients and their families about self-care methods and information related to their diagnosis.

• Accurately document care provided in a timely fashion.

• Work collaboratively with the care team, which includes OT, PT, HHA, and other disciplines, and relay patient needs to the RN or Clinical Supervisor.

• Utilize supplies and equipment safely, while maintaining aseptic technique.

• Undertake additional responsibilities as needed for patient services.

⛳️ Requirements

• Completion of an accredited practical or vocational nursing program.

• Possession of an active LPN or LVN license in California.

• A minimum of 3 years of nursing experience, with a preference for home health or med/surg backgrounds.

• Current CPR certification from AHA.

• Valid driver's license, reliable insured vehicle, and the ability to travel to patient residences.

• Strong communication abilities and a collaborative, patient-centered approach.
