Licensed Practical Nurse

This is a fully remote position, open to applicants in United States.

📋 Description

• Engage in outbound telephonic outreach to patients and offer personalized coaching and support for those managing chronic health conditions, including but not limited to type 2 diabetes, hypertension, and cardiovascular disease.

• Assist patients in implementing individualized care plans and achieving their health goals by delivering education and coaching centered around behavior modification, nutrition, physical activity, and self-management techniques, utilizing evidence-based methods such as motivational interviewing and SMART goals.

• Perform thorough assessments of patients' health status, lifestyle choices, dietary habits, and readiness for change.

• Provide education on disease management, medication compliance, symptom identification, and preventive strategies, while executing care delivery tasks in alignment with Cadence clinical policies and established protocols, under proper clinical oversight.

• Track patients' progress, adherence to treatment plans, and health outcomes through regular follow-ups and remote monitoring tools.

• Record patient interactions, outcomes, and follow-up strategies in the Cadence platform; identify obstacles to care (including social determinants) and connect patients with relevant resources or internal support channels.

• Collaborate in real-time with clinicians and patient support teams using tools such as the Cadence Platform, Google Workspace, and Slack, while providing feedback to enhance workflows in a dynamic, change-oriented environment.

⛳️ Requirements

• A valid compact multi-state LPN/LVN license in the state of residence is mandatory.

• A minimum of 5 years of clinical experience as a Licensed Practical Nurse is required.

• Readiness for remote work (including reliable high-speed internet) is essential.

• An active CA state LVN license is highly preferred.

• Proficiency in Spanish is strongly preferred.

• Experience in providing patient education and support to individuals managing chronic conditions is preferred.

• Familiarity with AI-assisted clinical tools (such as Gemini, Open Evidence, or similar platforms) and the capability to discuss credibly with providers and patients about where AI enhances care delivery and where human judgment is indispensable.
