Licensed Practical Nurse

This is a fully remote position, open to applicants in United States.

📋 Description

• Perform outbound telephonic outreach to patients and offer individualized coaching and support for those managing chronic conditions, such as type 2 diabetes, hypertension, and cardiovascular disease.

• Assist patients in implementing personalized care plans and achieving their objectives through education and coaching centered on behavior modification, nutrition, physical activity, and self-management.

• Apply evidence-based methodologies, including motivational interviewing and SMART goals.

• Conduct thorough assessments of patients' health status, lifestyle choices, nutritional practices, and their readiness for change.

• Provide education to patients regarding disease management, medication adherence, symptom recognition, and prevention strategies.

• Carry out care delivery tasks in accordance with Cadence clinical policies and established protocols under suitable clinical supervision.

• Track patient progress, adherence to treatment plans, and health outcomes through regular check-ins and remote monitoring tools.

• Record patient interactions, outcomes, and follow-up plans in the Cadence platform.

• Recognize barriers to care, including social needs, and link patients with resources or internal support pathways.

• Collaborate in real time with clinicians and patient support teams using the Cadence Platform, Google Workspace, and Slack.

• Provide feedback to enhance workflows.

⛳️ Requirements

• An active compact multi-state LPN/LVN license in the state of residence is mandatory.

• A minimum of 5 years of clinical experience as a Licensed Practical Nurse is essential.

• Readiness for remote work, including reliable high-speed WiFi, is necessary.

• An active CA state LVN license is highly desirable.

• Fluency in Spanish is strongly preferred.

• Experience in providing patient education and support for individuals managing chronic conditions is preferred.

• Proficiency with AI-assisted clinical tools such as Gemini, Open Evidence, or similar platforms is a plus.

• Ability to communicate effectively with providers and patients regarding the role of AI in care delivery and the importance of human judgment.
