This is a fully remote position, open to applicants in North Carolina.
• Perform thorough biopsychosocial evaluations to assess the needs, strengths, and risk factors of clients.
• Deliver individual, family, and group therapy utilizing evidence-based practices (CBT, DBT, trauma-informed care, motivational interviewing, etc.).
• Create and execute personalized treatment plans in collaboration with clients and the care team.
• Track and document client progress, modifying treatment plans as clinically necessary.
• Offer crisis intervention, safety planning, and stabilization services when required.
• Work alongside physicians, nurses, case managers, and community health workers to ensure integrated, holistic care.
• Link clients to community resources such as housing assistance, food security programs, transportation, legal aid, and benefits enrollment.
• Facilitate warm handoffs and referrals to specialized mental health, substance use, or medical services.
• Complete accurate and timely clinical documentation in the electronic health record (EHR) in line with organizational and regulatory standards.
• Uphold confidentiality and comply with HIPAA and relevant state regulations.
• Engage in quality improvement initiatives, case reviews, and outcome measurement activities.
• Foster trusting relationships with clients, families, and community partners from diverse cultural backgrounds.
• Take part in educational workshops.
• Contribute to a welcoming, culturally humble, and trauma-informed organizational atmosphere.
• Active Licensed Clinical Social Worker (LCSW) license in North Carolina.
• Over 3 years of post-licensure clinical experience (preferably in community mental health, integrated care, or similar environments).
• Ability to provide therapy independently within a supported care framework.
• Strong clinical judgment in assessment, diagnosis, and treatment planning.
• Clear, professional communication and collaboration with care teams.
• Familiarity with telehealth platforms and clinical documentation tools.
