This is a fully remote position, open to applicants in Ohio.

• Facilitate case consultations and engage in collaboration with interdisciplinary care teams.

• Perform case management by evaluating patient needs, coordinating care, and connecting patients to community resources.

• Lead group sessions that emphasize psychoeducation, stress management, mindfulness, and meditation practices.

• Conduct individual patient visits to offer emotional support, stress management techniques, psychoeducation, and coping strategies.

• Record patient interactions and manage follow-up care to promote positive health outcomes.

• Must possess an LCSW license with the ability to practice in NY, FL, and MI (candidates from PA, LA, and OH are also welcome).

• A minimum of 2 years of experience as a LCSW is required.

• Bachelor’s and Master’s degrees are mandatory.

• Experience in asynchronous patient communication is preferred.

• Familiarity with GI conditions or weight management is an advantage.

• Skilled in utilizing telehealth platforms and electronic health record (EHR) systems.

• A strong desire to assist clients in achieving behavior change by helping them establish health goals to effectively manage chronic diseases.

• Experience working with patients in a clinical setting is preferred.

• Demonstrates a commitment to exceeding standard responsibilities to ensure optimal care.

• Excellent skills in interpersonal communication and problem-solving are essential.

• Outstanding organizational abilities are required.

• Must have experience and the ability to excel in a fast-paced, high-growth environment.

• Strong personal computing skills are necessary, with remote work experience preferred.
