This is a fully remote position, open to applicants in North Carolina.

📋 Description

• Perform clinical evaluations, diagnose within your professional scope, and create cooperative treatment plans using approved formats.

• Deliver individual, group, and family interventions that incorporate evidence-based practices while adhering to ethical and legal standards.

• Ensure timely and compliant documentation, fulfilling supervision, training, and licensing obligations.

• Contribute to culturally aware therapeutic programs, engage in committee activities, and comply with organizational policies.

• Actively participate as a member of the PCHH team, supporting patient self-management objectives, proactive care, and case evaluations.

• Offer 1-2 available appointments daily for internal referrals within CommWell Health.


⛳️ Requirements

• Earned a master’s degree in counseling, psychology, social work, or another human services-related field from an accredited/approved college or university.

• Possession of a Licensed Psychological Associate (LPA), Licensed Marriage and Family Therapist (LMFT), Licensed Clinical Social Worker (LCSW), Licensed Clinical Addiction Specialist (LCAS), Licensed Professional Counselor (LPC), or Licensed Mental Health Counselor (LCMHC) credential, with eligibility for licensure within 24 months of the hiring date.

• Current CPR/AED/First Aid Certificate from the American Red Cross or American Heart Association, or a BLS Certificate along with a Basic First Aid Certificate from the same organizations.

• NCI+ Preventive Certification required.

• Minimum of one (1) year of clinical experience in a primary care, community health, or outpatient environment.

• Experience working with underserved or rural populations preferred.

• Knowledge of team-based care or PCMH/PCHH models is a plus.
